The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of the relevant rules, regulations, codes and guidelines. PSCO observed multiple non-conformities within the ship's SMS system. Recommend an ISM audit be carried out. The following was observed by the PSCO while on board the vessel: 1. Vessel failed to have oxygen content below 8% while at Anchorage 09 (Port of San Francisco) PSCO observed tank 01 port and starboard with oxygen readings at 10.5% and 9.8%. The vessel failed to follow internal SMS guidelines in regards to checking oxygen content of all cargo tanks. As per the QHSE Manual - Cargo Operational Manual (Oil Tankers), Section 5, the cargo tanks shall be checked for oxygen content two times per week. Records on board and Chief Officer’s own statement indicate that the last time oxygen content was checked on all cargo tanks was 08APR2021. 2) Procedures were not followed to secure the IGS lines to each perspective tanks after completion of a partial discharge of cargo. IGS isolation valves were kept open after cargo operations, which resulted in the IGS main line pressure alarm (set by IG manufacturer at 1200mmWG) activation. The crew used a paper clip to hold down the alarm acknowledgment button to silence the high pressure audible alarm and subsequently any other audible alarm produced on the IG alarm panel in the Cargo Control Room and on the Bridge. 3) As per QHSE Manual - Cargo Operations Manual (Oil Tanker)- Section 5, vessel shall have suitable connections for supply of inert gas. Isolation valve on the IG system was removed by crew and never replaced or reported to class. Valve is located in the vicinity of port side tank number 02. 4) As per QHSE Manual Section 10-Environmental Management, vessel shall obtain garbage record receipts from shore reception facilities. Vessel failed to obtain or use receipt template provided in Garbage Management Plan. 5) As per the ship’s SMS, the vessel shall follow the approved SMPEP plan. Vessel crew is not accurately inventorying all SMPEP locker response equipment. Multiple pieces of oil spill response equipment were not present on the vessel. 6) Vessel crew failed to follow on board housekeeping and safety procedures such as storage of cargo samples and use of non-sparking tools in hazardous areas.
