Description Due Date Resolved On ship carrying flammable products. A water-spray system for cooling, fire prevention and crew protection should be installed to cover exposed cargo tank domes. Any exposed parts of cargo tanks and cargo compressor room facing the cargo area. PSCO observed insufficient water spray coverage on #1 cargo tank dome. All 03 spray nozzles were clogged preventing adequate coverage. PSCO observed insufficient water spray coverage on the entire branch line on forward bulkhead of cargo compressor room. Vessel's crew unclogged all 03 spray nozzles over #1 cargo tank dome, and branch line. PSCO observed adequate coverage. Vessel's crew provided satisfactory water spray test conducted on 08MAY2022.

LOW-LOW PRESSURE ALARM WAS SUCCESSFULLY ACTIVATED AND COULD BE HEARD ON DECK, AFTER MULTIPLE ATTEMPTS, CREW COULD NOT DEMONSTRATE HIGH PRESSURE ALARM FOR CARGO TANK #1 AND #4, CREW STATED THIS SYSTEM WORKED WHEN TESTED A MONTH AGO.
